Treatment and patient responsibilities
Health care results are not 100% guaranteed. Results depend on the individual’s body’s reaction to the treatment, as well as your own behaviour (i.e. following instructions, coming for follow-ups). The practice can not be held liable for negative outcomes.
Health and pharmacy legislation prevents us from taking back any equipment we have provided to you. We can also not refund you in these circumstances. If there is a proven quality or performance fault with the goods (equipment, medicine, devices), we will contact the supplier, who will deal with the matter accordingly. If you have made alterations yourself, you may invalidate the warranty.
General rules and regulations
We do our best to attend to you on time; however if a previous patient requires longer care, or in case of emergencies, we might run a bit late. We will try our best to inform you on time. Should you need to reschedule your appointment please let us know at least 24 hours in advance. You will be held liable for short notice cancellations and no-shows.
You must adhere to the rules of the practice and any instructions given to you by staff or healthcare professionals. You have the right to ask questions and to have them answered. If you do not ask any questions, we will assume that you understand and agree to comply with everything.
You and those who accompany you to the practice should not harass the healthcare professionals and staff. Our staff must be treated with respect at all times. If not, we are allowed by law to refuse to treat, or to continue to treat you and your children. In such cases we will refer you to another practice.
Confidentiality and Personal Information
These terms and conditions informs you of our policies regarding the collection, use and disclosure of the Personal Information you provide. We use your Personal Information only for providing and improving our services and treatments. By “accepting” these terms, you agree to the collection and use of information in accordance with this policy.
The practice will protect all personal information to the extent required by law. We will use your information only in relation to your healthcare. If you’re between 12-18 years old, we will release information to family members only with your written consent. By law and if required, we will disclose your personal information to your medical aid (for benefits and coverage purposes), and healthcare professionals involved in your care.
We keep and may send anonymised information (i.e. without your name, identity number or address) to companies who collect this data to track trends in health care services. Some medical aids disclose dependents’ information to the main member of the medical aid scheme. We are not liable for any personal information disclosed as a result of this.
We may ask you to provide us with certain personally identifiable information that can be used to identify, contact, help with diagnosis and/or treatment. Personally identifiable information may include, but is not limited to your name, id number and various contact details (“Personal Information”). We may use your Personal Information to contact you with newsletters, marketing or promotional materials.
We may collect information that your browser sends whenever you complete this form online (“Log Data”). This Log Data may include information such as your computer’s Internet Protocol (“IP”) address, browser type, browser version, the time and date of your visit, the time spent on pages and other statistics. In addition, we may use third party services such as Google Analytics that collect, monitor and analyze this.
The security of your Personal Information is important to us, but remember that no method of transmission over the Internet, or method of electronic storage, is 100% secure. While we strive to use commercially acceptable means to protect your Personal Information, we cannot guarantee its absolute security.
